Rheumatological diseases bring together chronic, inflammatory and autoimmune conditions, including rheumatoid arthritis, ankylosing spondylitis, fibromyalgia, juvenile idiopathic arthritis and systemic lupus erythematosus. They often come with pain, joint stiffness, muscle weakness, reduced physical fitness, psychosocial difficulties and a lower quality of life. Medication remains the basis of management, but movement is increasingly recognised as an important complement.

In rheumatoid arthritis, Pilates programmes run a few times a week for eight weeks were associated with stronger muscles, less pain, greater muscular endurance and better quality of life. Other studies showed improvements in fatigue, mood and aerobic capacity, comparable to aerobic or combined exercise, which also hints at a possible effect on inflammatory processes.

For lupus and juvenile idiopathic arthritis the evidence is still limited but promising: women with lupus showed improvements in lipid profile and fatigue, while children with juvenile arthritis experienced Pilates as an enjoyable activity that helps control pain without excessive fatigue and supports balance and posture.
The authors conclude cautiously and practically: Pilates supports both the physical and the psychosocial side of living with rheumatological disease, and its low-to-moderate intensity encourages people to keep going. Larger studies with standardised protocols are still needed, and programmes should be individualised and supervised, with medical clearance.
